Ultrasound is an indispensable tool in prenatal care. When used judiciously and with clinical indication, it allows: Evaluation of fetal anatomy; Early identification of structural malformations; Growth monitoring; Support for fundamental medical decisions for the health of the pregnant woman and the baby. In recent years, we have seen an increase in demand for so-called " emotional ultrasounds ." hese ultrasounds are performed by technicians with the sole purpose of "seeing the baby" and obtaining beautiful 3D or 4D images. "Emotional ultrasounds" have no clinical purpose. Ultrasound is a form of energy that although considered safe when used appropriately, must follow precautionary guidelines. In medicine, the ALARA principle - As Low As Reasonably Achievable – applies, according to which exposure should be kept as low as reasonably possible to achieve a clear clinical objective. Obstetric ultrasounds, performed by physicians specialized in prenatal diagnosis, are part of structured clinical protocols. They are performed at specific times during pregnancy; They fulfill well-defined diagnostic objectives. Conversely, ultrasounds performed exclusively for recreational or emotional purposes expose the fetus to ultrasounds without any diagnostic purpose. Emotional ultrasounds do not replace obstetric ultrasounds in any way and may, in extreme cases, create a false sense of security. In this specific context, the Portuguese Entidade Reguladora da Saúde (ERS) recently issued an alert about the risks associated with performing obstetric ultrasounds not included in clinical protocols and emphasizes the importance of ultrasounds being performed only: Whit diagnosis purpose; By qualified physicians; In health institutions duly licensed to perform it; Always respecting the principles of safety and quality of healthcare. Health authorities, as well as several international scientific societies, converge on this guidance, reinforcing that ultrasound should be used as a means to promote maternal and fetal health and not as a consumer product. At Hospital da Luz, we advocate an approach grounded in scientific evidence, prioritizing safety and promoting health literacy. Pregnancy is a unique and emotionally significant phase, and its surveillance should be based on quality medical care, performed at the right time and with well-defined objectives. Informing, clarifying, and guiding pregnant women is part of our commitment.
